Balance by CCHP provides free aids and services to people with disabilities, including appropriate auxiliary aids and other services such as:
- Qualified sign language interpreters
- Written information in other formats (large print, audio, other formats)
Free language services are available to people whose primary language is not English, including electronic and translated documents and oral interpretation, such as:
- Qualified interpreters
- Information written in other languages

Data Updates and Discrepancies
This online directory is periodically updated. The availability of physicians, hospitals, providers, and services is subject to change. Practitioner information is either provided directly by the practitioner or obtained through the credentialing process.
If you want to report any inaccuracies in provider or facility information, please contact Provider Relations.

Balance by CCHP enrollees have full and equal access to covered services, including enrollees with disabilities as required under the federal Americans with Disabilities Act of 1990 and Section 504 of the Rehabilitation Act of 1973 and Section 1557 of the Affordable Care Act, and the Department of Health and Human Services implementing regulations at 45 CFR parts 84 and 92.
Reproductive Health Care
Some hospitals or providers may not offer certain services, including family planning, contraceptive services (such as emergency contraception), sterilization procedures (including tubal ligation during labor and delivery), infertility treatments, or abortion services. Case Management Program and Referral Information
CCHP Case Management Program
At CCHP, we’re committed to enhancing your healthcare experience and overall well-being.
This is a free program and will not affect your benefit. One of our nurses and social workers will work with you individually to support your health needs.
Our Care Management Program offers:
• Post discharge care management
• Chronic disease management
• Complex case management
• D-SNP case management
We can help accessing:
• Doctor’s appointment/ specialist referral
• Medication review
• Health education/coaching
• Transportation assistance
• IHSS application
• Meal Delivery
Members and providers may refer themselves or a patient to our Case Management Program by calling the Care Coordination Department at 628-228-3333 or by emailing [email protected].
Our intake coordinator will collect the necessary information and forward the referral, along with the identified needs, to one of our nurses or social workers for follow-up.
