Upon the end of the COVID-19 Public Health Emergency (PHE) declaration on May 11, 2023, CCHP has changed member benefits for COVID-19 testing. Most members still have wide access and coverage for detection and prevention of the transmission of COVID-19.  Please review details carefully below.

For Balance by CCHP members in Individual and Family, Covered California, and Employer Group plans:

  • No prior authorization will be necessary to obtain these tests.
  • COVID-19 tests can be obtained Over-The-Counter (OTC) at the authorized Balance by CCHP At-Home Test Provider Network. These include pharmacies, retail stores, and online retailers. Simply, show your Balance by CCHP Member ID card at check-out. (Find an in-network pharmacy through Pharmacy Locator) You may receive up to eight (8) tests every 30 calendar days.
  • Balance by CCHP will reimburse out-of-pocket costs to the member for up to eight (8) tests every 30 calendar days and up to $12/test.  Members may obtain reimbursement for out-of-pocket costs by submitting a completed reimbursement form, along with proof of purchase.

Balance by CCHP members may receive reimbursement for purchased COVID-19 at home tests.
