We are pleased to introduce our Complex Case Management Program, designed to support members with multiple chronic conditions—particularly those with uncontrolled Type II diabetes (HbA1c ≥ 8.0%) and heart failure. Participation in this program does not affect member benefits.
The program offers support in the following areas:
- Health education and coaching for management of Type II diabetes and heart failure
- Assistance with scheduling primary care and specialist appointments
- Medication education and review
- Coordination of transportation to medical appointments
